چکیده

Introduction MD is a bedside in vivo sampling technique that permits continuous analysis of a patient’s interstitial fluid chemistry. As the interstitial fluid bathes the cells, its composition reflects the local metabolic activity of those cells thus reflecting intracellular metabolic changes and disorders. The primary substances that provide information about metabolism are lactate, pyruvate, glycerol and glucose. In addition, blood metabolic alterations have been well characterized in severely ill patients. In vivo MD is performed by implanting a commercially available catheter that mimics a blood capillary at the site of interest.

Monitoring of glucose in brain, adipose tissue, and peripheral blood in patients with traumatic brain injury: a microdialysis study.

BACKGROUND Episodes of hyperglycemia are considered to be a secondary insult in traumatically brain-injured patients and have been shown to be associated with impaired outcome. Intensive insulin therapy to maintain a strict glucose level has been suggested to decrease morbidity and mortality in critically ill patients but this aggressive insulin treatment has been challenged. One aspect of stri...

Glycolysis in Patients with Sepsis during Early ICU Hospitalization Shows Differences vis-à-vis Shock Resolution – An Adipose Tissue Microdialysis Study

To assess early-onset differences in glucose metabolism in patients with sepsis during the first day of ICU hospitalization and evaluate these differences vis-à-vis shock resolution, we studied 10 (8 men; mean age+SD: 58+20 years) mechanically ventilated patients with a diagnosis of septic shock. On day 1 blood and microdialysis (MD) samples were collected twelve times per day (every 2 hours) f...

Year in review 2013: Critical Care - metabolism

Novel insights into the metabolic alterations of critical illness, including new findings on association between blood glucose at admission and poor outcome, were published in Critical Care in 2013. The role of diabetic status in the relation of the three domains of glycemic control (hyperglycemia, hypoglycemia, and glycemic variability) was clarified: the association between mean glucose, high...

Monitoring of Cerebral Blood Flow and Metabolism Bedside in Patients with Subarachnoid Hemorrhage – A Xenon-CT and Microdialysis Study

Cerebral ischemia is the leading cause of morbidity and mortality following aneurysmal subarachnoid hemorrhage (SAH). Although 70% of the patients show angiographic vasospasm only 30% develop symptomatic vasospasm defined as delayed cerebral ischemia (DCI). Early detection and management of reversible ischemia is of critical importance in patients with SAH. Using a bedside Xenon enhanced comput...

The Correlation between Cerebral Blood Flow Measured by Bedside Xenon-CT and Brain Chemistry Monitored by Microdialysis in the Acute Phase following Subarachnoid Hemorrhage

Cerebral microdialysis (MD) may be used in patients suffering from subarachnoid hemorrhage (SAH) to detect focal cerebral ischemia. The cerebral MD catheter is usually placed in the right frontal lobe and monitors the area surrounding the catheter. This generates the concern that a fall in cerebral blood flow (CBF) and ischemic events distant to the catheter may not be detected. We aimed to inv...
